Assessment at SCC utilizes the five-column form. This form calls upon faculty to identify program student learning outcomes or other pertinent measures that will be assessed throughout the annual reporting cycle. The first three columns are prepopulated according to each program's standing Assessment Plan, including:

  • Student Learning Outcomes
  • Means of Assessment
  • Criteria for Success

The final two columns are completed at the end of the annual reporting cycle cycle, including:

  • Assessment Results
  • Use of Results
