Forms can only be submitted by email in PDF format at this time. If submitted as an image (jpeg, png, tiff, etc.), you may be asked to resubmit your form as a PDF.
Please note, there are a number of apps available that allow you to take pictures of documents and turn them into searchable PDFs on your phone. Directions are available for using Notes on iPhones and GoogleDrive on Androids . In addition, Adobe Scan and Microsoft Office Lens are both free applications that are available for download in the Google Play and the App Store.
