From every college you attended after you left. Minnesota State transcripts are free for us to pull.

Appeal Form (only if you were suspended)

Send the Appeal Form after completing the re-admission process. Appeals are reviewed by committee in the order they arrive, and aren’t considered until the admissions process is complete.
